Manual lymphatic drainage (MLD) is one of the most misunderstood treatments in massage therapy. Many people have heard the term, but few understand what the lymphatic system actually does, why it sometimes needs support, and what to expect from a proper MLD session.

As a therapist trained specifically in MLD, I want to demystify this powerful treatment — because for the clients who need it, it can make a profound difference.

What Is the Lymphatic System?

The lymphatic system is a network of vessels, nodes and organs that runs throughout your entire body. It performs several critical functions:

  • Removing waste products, toxins and excess fluid from body tissues
  • Transporting immune cells that fight infection and disease
  • Absorbing dietary fats from the digestive tract
  • Maintaining fluid balance between blood and tissues

Unlike your circulatory system, which has the heart as a pump, the lymphatic system has no dedicated pump. It relies on muscle movement, breathing and body motion to keep lymph fluid circulating. This makes it vulnerable to sluggishness — particularly after illness, surgery, inactivity, or certain medical conditions.

What Is Manual Lymphatic Drainage?

MLD is a specialised massage technique that uses very light, rhythmic strokes to stimulate the movement of lymph fluid through the body. The pressure used is far lighter than in a Swedish or deep tissue massage — we're not working muscle, we're working the superficial lymph vessels just beneath the skin.

Sessions are slow, methodical and deeply calming. Many clients fall asleep.

Who Benefits from MLD?

MLD has a wide range of applications, from medical to wellness:

Post-surgical swelling

After surgery, lymphatic drainage is often disrupted, leading to swelling (oedema) around the site. MLD can significantly speed up the resolution of post-operative swelling and bruising. It's commonly used after cosmetic surgery (liposuction, tummy tuck, Brazilian butt lift), joint replacements, and other procedures.

Lymphoedema management

Lymphoedema — chronic swelling caused by damage to or removal of lymph nodes — requires ongoing management. MLD is a core component of decongestive lymphatic therapy (DLT), the standard of care for lymphoedema. I work with clients managing lymphoedema as part of their broader care plan.

Post-cancer treatment

Lymph node removal or damage as part of cancer treatment (particularly breast cancer) frequently leads to lymphoedema in the arm, breast or chest wall. I am trained in oncology massage and work with cancer-affected clients with great care and sensitivity.

General health and wellness

Even without a specific medical indication, MLD is deeply relaxing and supports immune function. Many clients book regular sessions as part of a general wellness routine — particularly during periods of stress, when immune function may be compromised.

The lymphatic system is the body's waste management network. When it functions well, you feel it — in your energy levels, your skin, your immune resilience.

What to Expect in a Session

A typical MLD session begins with a health consultation to understand your symptoms and goals. I will then work systematically through the body using the characteristic light, rhythmic strokes. Sessions usually last 60–90 minutes depending on the area being treated.

Afterwards, many clients feel a strong urge to urinate as the body clears excess fluid. You may feel lighter, less puffy, and deeply relaxed. Drinking plenty of water after a session supports the process.

Important: MLD Is Not Deep Tissue Massage

A common misconception is that MLD is simply a lighter Swedish massage. It is not — it is a clinically distinct technique requiring specific training. If you've been told you need lymphatic drainage, please ensure your therapist has dedicated MLD training. I do.
